I've recently got out of the Army and has couple of years of medical with the Veteran Affairs. I'm trying to research what an AME is required to fill out for a 3rd class medical because my VA health physician had completed some several years back and haven't been keeping up with the changes. Trying to keep it in the VA because all my health record is through them and don't want to do more work going back and forth with civilian offices. I tried google and only found that FAA Form 8500-8 is required.

What kind of paperwork is required to complete a 3rd flight physical and where can I get them? Any comment is appreciated
