Sec 25.H.3.b.ii pertains to an election in the first Sub window. If no trip is offered during that window, he may elect OTP for the remainder of the Sub Window, or he may elect to remain in substitution.

The question posed assumes that he has remained in substitution, so the OTP election is gone. It's no longer an option. If he refuses a sub trip in a subsequent Sub Window, the entire Sub guarantee is forfeited. (Sec 25.H.6.iii.(b).)

Again, on the "lengthened" question. The Sub window ends 4 hours after the original trip ended. The showtime and scheduled termination of a substitution trip must be within a pilot’s substitution window. That's the limit of how much the trip can be extended -- 4 hours.
