Read the ops spec B046 that goes along with the LOA. It contains text which states who the RVSM training is provided by. King Schools is a very common vendor. There is no recurrent training requirement for Part 91 ops.
Not necessarily true. Everything depends on how your RVSM manual is written and the details of your LOA.
