I believe the Pinnacle RJ crash could have been partially attributued to a low time CA and FO (as well as both being stupid). Didn't they take the AC up to 41,000 on a ferry flight, and the FO stalled it while the CA was in the back grabbing a coke. They couldn't restart the engines due to lack of system knowledge (and core lock), and crashed just a few miles short of the runway while trying to deadstick it in.

Excellent example.. a low time (or at least Jet experience) RJ CA, with little understanding of high altitude aerodynamics and jet performance was probably the key cause...
