Mach84 and junior, I am in complete agreement with both of your examples. I would like to add my current pet peeve, which is pilots that position their boom microphone in front of their mouth, resulting in constant "popping" or air noises when transmitting. It sounds like they're transmitting from inside a wind tunnel at times.

To those who may be guilty of this fauxpaux, please position the boom microphone properly to avoid this highly annoying (but way too common) occurrence.
