RE. the crashed 777 in SFO, sounds like they were trasitioned to the inbord (28R) on final. WX looked good, but eye witness said they were low and slow (obviously) and was the GS up for the inboard?

I don't know if that one had the Rolls Royce engines, but several years ago there were issues with the fuel/oil heat exchanger, forming ice balls, which resulted in the BA 777 landing short. They pushed the power up and nothing happened, due to the ice in the fuel line. They were all supposed to be fixed several years ago.
