For phone numbers, just ask ground to give you the tower number. I doubt they'll call you if you give them a number, but hey try it. So, call the tower and ask to speak with the tower supervisor.

If your airline has FSAP program file a report NOW. Otherwise file a NASA report, NOW. If you have union representation, call your saftey rep, NOW.
