Pilot House shirts are my current favorite (pilotshirts.com). Tapered, made in Canada. Good quality, and stand up well to repeated washing.
2nd choice Crewgear shirts. They also have a trim fit and good material. They used to be made in the USA, I believe that is no longer the case. They don't seem to last quite as long.
Van Heusen is terrible. They look OK for a few weeks and then the thin material basically disintegrates from the washing machine. They also don't fit well on an athletic person. Stay far away.
I've heard good things about Landerwood shirts, made in the USA, but they are not cheap and I haven't personally tried them.
