My understanding is they are asking for all failures. I know a guy that double busted his part 121 upgrade and was still hired at Delta. Being honest is the best policy.
I have met quite a few guys at Endeavor who have double busted the upgrade. One guy busted so many times trying to upgrade they made him take a 709 ride. He passed the ride and was awarded his 4th strip. That should be fun to explain during his SSP interview.
