If it's not on your driving record then don't put it on the application. Your driving record is all the airlines are going to see. I think most airlines are only concerned with a 5 year history for minor things like speeding tickets anyway.
That is kind of what I gathered. I can't pay for a super screener life history report and I am not going to list a ticket because there MAY be one from 10 years ago that I can't remember.
