They do both depending on need. Both have their upsides. If the Captain has been on the airplane, that's a plus. However, he's probably not going to want to study flows with you, etc ....
I'd prefer FO-FO for initial since you can concentrate on FO stuff and help each other. After you are familiar CA-FO works perfect.
I've done both on an initial course. The CA-FO was on an AQP program. I preferred the CA-FO program, especially since it was my first 121 gig. The CA had ZERO problems helping me out on stuff. From flows to just regular operational things that made the LOFTs a easier.
But he was a GREAT CA. If you get paired with crotchety old bastard that thinks a newhire could "sink" him and considers it a liability, may not bode so well.
