Yes it can happen, and it does happen. But why does it happen when there are two fully qualfied airmen, and sometimes three or four in the cockpit or a modern day jet with convential avionics to back up the automation?
Why not ask JVvZ, the 747 Chief Flight Instructor for a certain Royal airline headquartered in Amstelveen. He was so respected that management went looking for him to investigate (what was unbeknownst at the time) the crash that caused his own death.
