There's a couple guys in SLC that taxi SE and shut the APU down as soon as you get #2 started, then ask you to do a cross-bleed start. Both of them are, of course, LCA. There's no-one as non-standard and risk-accepting as an LCA, in my experience.
Just out of curiosity, did you, at any time, tell them "Hey I'm feeling rushed and behind, do you mind if we crank it up now?"
I don't mean to be a tool about it, but the reason these guys do this is because there's no push back.
