Yeah, you did.
All you need to do is go to the Fidelity site, on the left side of the page under "Act" go to "Contribution Amount" then "Contribution Amount and Catch-up Contributions." Scroll down to the "Profit Sharing" contribution and you can put in a percentage. You have to do it by 4pm on January 22, 2014.
