No, I don't think so. Delta asked me to bring copies of all my 121 checks "if available", so I got my company to send me copies of all my sim and line checks. They'll get the same forms on the PRIA check, so just answer the question honestly if (when) they ask you if you failed a ride. Not a big deal if you disclosed it on the application and they hire you anyway; a very big deal when they find it on the PRIA check and it's not reflected on the application.
Thanks germb74 - I appreciate the quick reply. And congrats on the Delta interview!! I'm heading to a Frontier interview soon myself. I have my original GA log book with endorsements, another one that looks just like it (hand written) with all my 121 FO time, and then I went digital for all my CA time. Followed the advice of the gentleman above about having it printed and bound at Kinko's (fed ex office) and it looks pretty decent.
