I'm not an MD/AME but this is how I understand it...

The third class has some significant differences in vision standards (and maybe hearing?).

The differences between 1st and 2nd are not really spelled out, so for purposes of your annual (or bi-annual) exam there are really no differences. The actual difference is determined by the way the FAA evaluates a problem medical condition, if one exists.

Let's say you have a condition that precludes both 1st. and 2nd class certification...the docs in OKC may be more willing to give you a waiver for a 2nd. class rather than a 1st. class. So you could still fly as an airline FO, but not a CA.
