I think the guys who couldn't, have; too late to save some of their passengers. Of course the correct autothrottle mode would have given them a heads up about a minute earlier. A good pilot isn't afraid to use all of his systems. Of course if you are one of those guys who never makes errors let the other guy know before block out, he may want to back out of this trip.
Clearly you are not reading what I have said. Never once did I say to fly every single approach manually (even though that is my preferred method). I said don't muck around with partial automation. AP off - AT off. If, for whatever reason you don't want to turn off the AT then leave the AP on, if you're 'too tired' to move the throttle levers you're 'too tired' to move the yoke as well.
