I think the following is what I do for LBP printing but I haven't had cause to print it in quite some time so I may have forgotten a detail or two. From the menu bar I use Reports > Flight Log > Split Reports (Left/Right Segmented) > Commander & Full Series Binders > Custom Template. The custom template allows me to modify the columns. For example, under Aircraft Category & Class I've replaced columns for Helicopter, Glider, etc. with Turboprop and Jet columns. You can change the title of each column and the associated data field.
After selecting a custom template (or the default) you'll be prompted for date range. I use "all data" every time, even if only printing out the last few pages, to preserve correct page numbering. I then check page margins, which must be done under the "Left pages" and "Right pages" separately. Select the "Left pages" tab at the bottom of the report then select File > Page Setup. My current margins are 0.5" top and bottom, .75" left, and 1.0" right. The extra space on the right side is for binding and the narrower top and bottom margins allow a few more lines per page. Under the "Right pages" Page Setup I have the same top and bottom margins but left is 1.25" and right is 0.583". I don't quite recall how I came up with that but the finished product looks correctly spaced at the edges and the binding. YMMV depending on what kind of binder you intend to use. For Kinko's spiral binding the defaults may work just fine.
Next, I use the menu par option "Insert Cover Page & Binder Transitions" to insert a blank cover page. This sets the pages up correctly for Duplex printing. Otherwise page 2 will be on the back of page 1 and you won't have the correct halves of each entry lined up. For some reason the program recommends swapping the page margins when I do this, but this is a bad idea. Doing so shifts everything in towards the binding, so I select "No" on this prompt. You can also right click and select "Report Options" to select whether you want page numbers displayed odd rows shaded. My last print job used shaded odd rows and shading is very inconsistent. I'm contemplating printing without that next time.
Once everything looks right I select File > Export > Export to Acrobat Reader (.pdf) to save the formatted report. I'll usually test a couple pages to make sure I didn't screw something up. Once satisfied I take the file to Kinko's for a big print job and to get holes drilled in the pages to fit the binder I use. If it is just a few supplemental pages I'll do that at home by producing a similar pdf and just printing the needed pages.
Lastly I print a custom cover page on the blank reverse of page 1 and add pages at the back with scans of all of my personal and student endorsements. Hopefully some of that is helpful. There may be easier ways to go about it but this is what worked best for me after much trial and error.
