I had a discussion with the ATL Int. Chief Pilot about this very topic a few years ago. He asked me if I had my Malarone prescription.

I said, "Nope, I'm never going there, so I don't need it."

He said, "Well, what if they cancel your Dubai trip and assign you a Lagos trip for recovery?"

I said, "That's what sick leave is for, I'll just sick out of it BEFORE I go and get Malaria, so I'll save Delta a bundle in both Malarone prescriptions and malaria treatment costs."

No argument from him. BTW, he'd never been there either and had no desire to go, and this was BEFORE the Ebola thing!

You can refuse to do anything you want. You're not going to have any contractual protection. It's recommended to start Malerone 2 days prior however the CDC states that if you start it 8 hours before entering a malaria zone you should have full coverage.
