I know of one guy who turned on unprotected side of a hold and another who tracked outbound from the VOR for 5 min before figuring it out. Both were hired.......
I did the ILS 06 KTTN. My clearance was cleared direct Yardley VOR, cleared ILS 06. I hit the VOR and turned outbound on the feeder radial. By the time I got established on that radial I was about 2 miles past ZUBAX where I was supposed to start the hold. What saved me is that the 222 radial from Yardley is pretty close to the right heading for a teardrop entry. Once I realized I was already 2 miles past the fix I just started my inbound turn on the teardrop and managed to get myself on course and shoot the approach. I was sweating a bit though.
I also go about 300 ft off altitude at one point when I was looking down at my chart. If they let you see the approach before hand try your best to memorize everything you can about it so you don't have to look down. The sim is really touchy.
The guy interviewing me didn't say anything about my approach or getting off altitude. I think everyone messes something up in the sim. They're testing to make sure you recover from your mistakes and keep your cool.
