Years ago, at American, my wife was working First Class on the DC-10. Agent's in a rush, wife says we can't go, agent closes the door.
No Pilots on board. 
Had an agent tell a former Southern captain she was in charge of the door and shut it. We sat for 2 hours. No fuel slip.
