It's been awhile, but what I've done in the past is
scan the Jury Summons and e-mail it to the CPSC and CPO. A follow-up phone call couldn't hurt. Treat it like a hot potato.

I've heard of guys faxing documents in and, due to Murphy's Law, the documents were inadvertently fed in upside down. They got a confirmation that X number of pages were successfully sent. Unfortunately they were all blank pages.
If I'm sending something to the company (Jury Summons, Medical Certificate, Sick Verification, etc) I scan it and email it with a BCC to myself. That gives me a clear record of what I sent, when I sent it and who specifically I sent it to.
