I would fill it out anyways, even if the FAA doesn't have a record for you, they're asking for it. To easy to just sign and have a complete file, than have them ask you later after you've been put in the bottom of the stack.
Mil duty is exempt from PRIA... Google the PRIA reg... And control + F military. It's in there... Not required.
