It didn't happen to Bernouli so I guess it doesn't happen and never happened. Give me the 100 best pilots in aviation and I can get 95 of them to fail a check ride in a sim on a single maneuver. And all 95 will agree that they failed. Nothing hard but you set them up mentally to expect one thing and something else happens. The loud noise along with the timing starts a mental process that causes the error. I bet every sim instructor has a similar item that they discovered by accident.
I remember at one airline the pilots were having the captains log all landings so that every FO would be noncurrent in 90 days. They were trying to force the company to do something, I forget what. The DO said that every FO who has to go into the sim to get recurrent could expect him to be there sitting in the sim. He didn't say he would make the instructor fail the guy but the instructor would not have any leeway to pass him if he wasn't perfect so both the instructor and the FO would be nervous. The job action ended quickly.
I remember another job action at a company. Eight guys in a row failed the sim and the job action abruptly ended. This was pre pria so it didn't have much effect except losing a little pay. With pria that kind of stuff can end your career. Actually, it may prevent you from leaving that company because others may not hire you. Now there is a creative way to keep pilots.
I recommend to go for the quality regionals. Comair is gone but Skywest is still out there. What other regionals would you consider quality?
