Well you said it best: MOST have years of 121 experience and MANY of them have thousands of hours of PIC...had your examples used the description "all" then these events would probably be reduced (not eliminated I might add...experience, as I imagine you know, doesn't always bring good judgement).

Our current upgrade time is not a liability and I apologize if my post alluded to that...the above scenario happens with 15-year upgrades as well. My statement was more focused on the frequency of how many of these cases pop up, which will obviously increase with quicker (read: more) upgrades. There were years in the last decade where we had 8-12 upgrades a year- not month- and a couple of these cases popped up...but not with near the frequency because there were simply so few new captains every year.

I think most of our new captains do an outstanding job of preparing for and transitioning to their new role, especially considering our sometimes challenging environments and minimal support. But let's be honest: our current training/checking curriculum does little to evaluate a pilot's judgment and decision-making and focuses instead on only minimal tasks per 121.441. Yes, AQP will help but that's still years away. I've had numerous conversations with FOs about "nightmare" captains and the wacky stuff they pulled; when I asked if they considered calling Pro Standards they looked at me with a blank stare. If we won't police our own who will?
