Hmmm, in the FAA medical it asks you to put down any visits to a medical professional within the last 3 years. Well what about if tomorrow they asked you to list it since age 18, would you suddenly remember that too? I bet you would there wouldn't you (:
No notebooks. But with a few moments to sit down with a paper and pen, I'm pretty sure I remembered them all. And I had more than my share.
If you can't remember who wrote you a paycheck in the past 20-30 years, maybe you *should* be talking to your AME.
Nobody's making you write down all this info. It's all voluntary.
