At spirit we accrue 4hrs per month bankable up to 400 I think. It can be used for yourself or to take care of a loved one. You can go negative in your bank by 25 hrs. If you separate from the company and have a negative balance it comes out of you last check. If you have a positive balance when you retire (not resign) it's paid at a percentage of your pay rate when you retire based on longevity. I think the highest is 50% for 15+ish years of service. I'd have to verify those numbers for accuracy.
I hope we see a large increase in accrual because 4hrs is quite inadequate.
We also don't have any verification language but I have heard of pilots getting quick calls from the chief pilot if they called in sick within 2hrs of show, even if it was a reserve period with no trips awarded yet.
