I've never seen any different "do-not-disturb" signs than the hotel issued versions but some guys/gals will carry an extra sign for the specific hotel, for example in Cologne, because the sign might be missing when they check into their room.
So rather than calling front desk for a new one they'll use their "personal" sign and will then take it with them when checking out.
I've also seen some people taping their do-not-disturb sign to the door as opposed to hanging it on the door handle. I assume it's to make it harder for the neighbor next door from stealing their "don't disturb" sign. It has happened to me before. Apparently some people are too busy to call front desk and request a new one.. :-(
