I went back and sifted through the emails and it looks like the form is titled "AUTHORIZATION FOR RELEASE OF EMPLOYMENT RECORDS RELATED TO ATTENDANCE AND RELIABILITY" and is included in the packet of documents and checklists of items to bring that they sent about a week prior to my interview.
Sure, you signed the form but did your previous company send anything? I bet not. I have it on fairly good authority that mine does not. They aren't required to and it takes man hours.
