For civilians this might be a good idea, for military just go to immunizations and get it for free. Tell them you need an international immunization certificate and you'll be g2g.
If your base doesn't have the yellow cards (like mine didn't), you can try the county health department. I stopped by and they gave me a few of them...went back to base and had them transcribe my immunization record.
The form name is CDC-731 (formerly PHS-731)
