Sent an email to the chief pilot that stated,
"Dear Chief Pilot,
Here is my two week notice of resignation.
Good day,
Captain Regular Pilot"

Doesn't have to be any more complicated than that. Your FOM may have guidelines of who to notify and what timeline is necessary. Congrats on getting out.
