The second question is easy. 64. Many times.
It has not been rare for 20,000+ hour pilots, with deep wells of experience and excellent qualifications, to finish their careers in the right seat. This has been a brutal business over the past 35 years.
Hopefully the non tie wearing generation will have a better ride.
