I would hesitate to put "Yes" to the third question.
You were pulled from a trip and given a second line check which you passed.
Being "Removed from Flight Status" typically involves paper work in which you have a "Status Change".
I would say "No" to that particular question. You still maintained flying status, you just had to update your Jepps.
