If you're medical is current when you report for indoc, you're good (based on the "current medical" requirement when I went through; there was no "current through xxx date" stipulation; YMMV if things have changed). Any medical you get subsequent to being on property will be reimbursed at the flat rate.

One caveat (I don't know your exact dates): figure out how much of a PITA it will be to get a medical done while you're in indoc and/or training--it may be worth it to get it done out of pocket ahead of time. Your call....

Sort-of. You get the flat-rate reimbursement when you submit your new medical certificate. You get the "higher" rate for the first one of the calendar year (to cover the EKG), the "lower" rate for the second one (if applicable, if your category and/or age require you to be on the six-month plan).
