I would recommend contacting Dr. Bruce Chien. He is a senior AME / HIMS specialist that deals with difficult certification cases ALL the time. Although he will cost something, it will be nowhere near what some of the other companies charge. Just google his name and you will find his contact info. He will tell you exactly what you need to do before you apply for a medical and he knows what he is talking about.
