It's generally when something out of the ordinary is said by a controller.
Example:
Other day in CLT it was busy with a long line waiting to cross the center runway. Controller says something along the lines of "I'm gonna cross everyone and I'm gonna do it fast please only respond with your call sign so we can get everyone going". "12345 cross 36C E, E14 to the ramp call sign only" 12345 in a THICK accent reads the whole thing back. She cross 3 or 4 more acft without issue, then another guy in a THICK accent reads the whole thing back.
Kinda threw off what the controller was doing and probably ended up getting someone stuck on the other side that would've otherwise been crossed because it took a little too long. Is it a huge deal? No. But calling someone out for telling it how it is a little over sensitive, and to deny that it happens is naive.
