Miami Air generally buys the cheapest ticket they can find for getting you from where you are to where you need to go. They also tend to not buy the ticket until the last minute in case the schedule changes. So the flights are usually not direct, and usually in economy. It's up to you to get your hands on a non-middle seat by whatever means possible. (You'll spend a good amount of time checking the seat map over and over the night before.) Sometimes you're lucky, sometimes your not.
Every once in a while the only tickets available are non-economy seats, but that is a rare occasion. They usually find other routings first.

It's charter... so it's not like there is a typical route. Go to priceline or whatever site you like and look at the cheapest routing from "A" to "B" (no limit on number of legs or limos) and that is what a deadhead looks like. Miami to Los Angeles could be something like: MIA-FLL (via van), FLL-CLT-PHX-LAX (via American). 19 hour duty day! International is the same story.....

Not sure what your question is about the double crew. On augmented flights, the jumpseat is occupied below 18,000 feet. Then in cruise there should be a crew rest seat in the back for the resting pilot(s).
