On my app I put the number of points received. i.e. "2 points" and "no points". They are going to order a national driver record check, so I ordered my state records first in order to make sure that the infraction and disposition that I put on the app matched the records. My 2 infractions were more than 10 years ago, so I didn't even remember the exact infraction. Also, there are airlines that will ask you to order your own records and submit them with your app. Netjets for example.
