Medical:
Everything is network based but with provisions to use out of network if needed. You can have a Preferred Provider Network or a preferred Physician network via Aetna, BCBS, CIGNA, or United HC - "These were created to best accommodate people living in different areas of the country." Primary Care Provider = must see primary care Dr to get referred to what you need, but these offer the better coverage. PPN = go where you wish in network. There are 4 total options you can choose depending on your needs.
Deductables (Plan): none; out of network $250/person 500/family.
Out of Pocket Max: $1,000/person, $3K out of network.
Lifetime max = $2mil/person w/ $1,000 per year of service to be restored.
Physician Office visit, Routine OBGYN, Preventative care & Mammograms, Allergy testing and treatment, Maternity, Rehab : $10/visit or 90% covered; 80% after deductible if out of network (OON).
In-Hospital: 85-100% depending on plan, 80% OON after deductible.
Surgery: 100%, 80% after deductible OON.
XRAY & Labs - either included in copay or token fee. 80% OON.
Well care child - $10/visit then 100%, *no out of network!!*
Chiropractic: Similar to Physician Office visit
Hospital admission Fee: $100, $250 OON
Inpatient/Outpatient Benefits: 100% / 80% OON
ER / Ambulance: 100%. $100 copay if ER used for non emergency issue.
Hospice: 100% / 80% OON
Skilled Nursing Facility: 100% / 80% OON
Outpatient private nursing / Home health care: 85-100%, 80%OON
Medical equipment: 90-100%, 80% OON
Mental Health - covered as above with small token fees.
PRESCRIPTIONS: $0 mail order, $0 generic non mail order, $5 non generic.
Yearly cost out of pocket for family of 4 = ($2000 - $4,000 depending on plan choice) or $90 to $160 per pay check. Sorry, I don't have the rate sheet, just going by what I pay.

Dental: No deductible, Basic (minor to include RCT) and preventative work covered at 100% no copay, Major work at 80%, Orthodontia at 50%. $300 something per year family of 4. PPO Aetna

Vision: Everyone gets routine eye exams free. Cheap plan if you need glasses.
Life Ins- Through UPS is $560/yr (tobacco free) for $1M for employee, $50K spouse for $26/yr. Kids $10K for $10/yr.
AD&D: UPS gives $100K free, you can go up to $1M for $250/yr.
Short Term Disability: $0 cost. Pays $185/week for up to 26 weeks but takes a little time to start.
Loss of Medical ins: 66% guarantee for 20 pay checks (9 months). Health benefits 2 years, up to 5 yrs if no other employment.
Health Care Spending Accounts - yes.
Work Life Benefits / mental, drug programs covered.
