Format your document to nearly match the supplied paper with box. Put a check mark on the front left corner of your paper in the tray (to know how your printer feeds the paper), and print it. Hold printed document behind the box and see if ti fits. Fix if doesn’t. Put the “boxed paper” in the tray, then print. Voila.

Literally eyeballed the format, and first print fit perfectly. It’s easy. Only used one sheet of paper.

Writing on the paper is probably ok, but they would have forwarded my application to the hospital...
