$411 or $909 a month for a family depending on the plan you choose (HSA or PPO). Worst case scenario (were talking catostphic illness) with the HSA you pay a $6000 out of pocket maximum and $5000 in premiums for the year, less $1350 in company contributions. That about equals the premium for our PPO plan which is very expensive.
