I have done this at my regular doc and always had it covered at 100%. Under 40, all that. Your doctor’s office probably coded it incorrectly, or ordered some extra testing that wasn’t necessary. Also, the conspiracy theorist in me believes that insurance companies “accidentally” bill for unnecessary reasons with the hope we’ll just roll over and pay.
