Has anyone ever had a junior assignment or extension just show up on your schedule without a proper notification?

Had that happen to me and I didn't notice till after my original pairing was over. I called after I was off duty and SOC admitted they had no clue why that happened. Was wondering if this was an one-off thing or other people are experiencing it.
