In the question about the drunk captain, I think another key part of the answer is that you should take the Captain aside somewhere private and question him about it, don't do all of this in the gate area where you guys are in view of all the passangers. Because most likely things could get out of hand, especially if the Captain doesn't take lightly to his young FO questioning his ability. Also definitely if he insists on getting on the flight, call a supervisor and most definitely don't get on the airplane.
