The test is given by one of the nurse assistants, it uses one of those DMV style machines that you put your head against and it lights up.
You're asked to read the letters on a standard looking eye chart, the smallest row you can read. Then another slide of the same, I assume testing near and far vision.
Then there are numbered music notes with a red line going through one, testing astigmatism/lazy eye/eye position I think.
Then there are 6 Ishihara plates to read the numbers out of for color vision. He also has a Farnsworth Lantern Test for color vision. I don't have great color vision and the Farnsworth machines are hard to find so I'm glad he has one. I've moved away from Atlanta about 5 years ago and I still go to Dr. Faulkner. I couldn't ask for a better AME.
