Ok, this is a first for me. OMG! I'm trying get a reimbursement from my Limited Purpose FSA for two separate crowns my wife got this year. I mailed in the first claim with copies of both EOB's. I get the response that my claim was denied: "The information you sent is unreadable, illegible or a corrupt file. Please resubmit." So the next time I submit my claim electronically with photo's of the EOB's. I just got the same denial reason!
Are you kidding me! They can't read a copy of EOB made with a copy machine or they can't look a picture of the same thing? Crowns aren't cheap and I don't want to forfeit money over the $500 carryover allowed.
This is total BS! This is MY money, NOT UHC's! My appeal letter is going out tomorrow. I'd call but of course they don't provide a phone number......the rat bastids!
I've never had a claim for reimbursement from my LPFSA denied before. Un ******* believable!
Anybody else have/had this problem?
