Or is it something people just have been doing over time, thus the status quo? I don't think it is a regulatory thing that an extension refusal has to go with a fatigue call/report. FAA leaves how extensions are handled to be worked out between the unions and companies Envoy thing or most others do this? It's a regional thing. Mainline you just go home no questions asked.
