Under AQP busting one maneuver normally does not mean you fail. If there is time left during your validation they can retrain you and let you do it again, up to a certain number. Crashing, however, normally means you failed.
This depends on the implementation. Our initial MV AQP is not train to proficiency, it's the usual 2 or 3 strikes and you're out, but no retraining.
Recurrent is train to proficiency.
