This sounds like something new. I will say the rate of folks failing training has gone up so they may have added a final review to curtail some of that. By far the guys that have been active CFIs are doing well. The failures are more common with guys that have decided to build time in their own aircraft and have never really gone to work every day as a pilot. The banner tow, pipeline, ag pilots are getting through but seem to be struggling more with ATC communication, etc on OE. If you have been a regular “in the system” you should be fine.
I’m guessing this is something to do with the final review. Happens when FAs are interviewing pilots and hiring everyone who walks through the door.
I do know the precourse that's been mentioned on here is not for everyone. You have to score lower in the interview to be sent to it. Honestly I asked if I could attend and was told no since I scored over the threshold in the interview. I just want every advantage I can get but oh well.
