One of the questions on the app is, "Have you ever failed ANY checkrides (military or civilian) or received an unsat (military or civilian) on ANY phase check, proficiency check, FAA check ride, line check or ANY portion of flight or simulator training (military or civilian)?"

I've never had a failure on a check ride or prog check, but I honestly can't remember if seventeen years ago I received an "unsat" on any particular objective of a flight lesson. I would assume I did at some point, but I don't have any records of that. How should I answer this question?
